Plain answers about the website, kept separate from the HIPAA notice that governs your clinical record.

This page covers the website. It explains what happens to information you send through apex-mhc.com before you are a client.
Your clinical record is a separate matter governed by HIPAA, Colorado law, and the Notice of Privacy Practices you receive and sign at intake. That notice, not this page, governs your protected health information. If the two ever appear to conflict, the Notice of Privacy Practices controls.
Only what you type into the contact form: your name, the contact details you choose to give, your stated preference for how and when to be reached, and whatever you write in the message field. That is sent to Dr. Reichert and is not published, sold, rented, or shared for marketing by anyone.
This site sets no advertising cookies and runs no advertising or social media trackers. It does not build a profile of you. Like nearly all websites, the server that delivers these pages keeps standard request logs, which include IP addresses, for security and reliability.
Email and web forms are convenient. They are not secure, and no ordinary email system should be treated as confidential. Send enough to arrange a call. Keep clinical detail for the call itself, where it belongs and where it is protected.
The same goes for text messages. If you text the practice number, treat it as you would a postcard.
If you do not become a client, the inquiry is deleted once the correspondence is clearly finished. If you do, the relevant portion becomes part of your clinical record and is retained for the period Colorado law requires for a licensed professional counselor.
The practice uses ordinary business services to operate: a web host, an email provider, and scheduling and records software. They handle data in the course of providing those services and are not permitted to use it for their own purposes. Where a vendor handles protected health information, a business associate agreement is in place as HIPAA requires.
Once you are a client, what you say is confidential with a small set of exceptions, all of which are required by law rather than chosen by the practice. They are discussed fully at intake. In short, disclosure may be required when there is:
If a situation like this arises, you will be told what is being disclosed and to whom wherever it is safe and lawful to do so.
You may ask what the practice holds about you, ask for a correction, ask for a copy, or ask that a website inquiry be deleted. Rights over your clinical record are broader and are described in the Notice of Privacy Practices. Write to info@apex-mhc.com or call 720 318 5833.
This practice serves adults and couples eighteen and over. The site is not directed at children and does not knowingly collect information from them.
